Our office is very sensitive to the costs associated with orthodontic treatment
and we are happy to work with you to arrive at an affordable monthly payment plan.
The treatment fees are paid over the treatment time on a monthly basis to make
it easier to fit the family bugdet. Please note that
the monthly fee is not an appointment fee. The monthly fee is due whether the
patient is seen once during the month, three times or not at all. Since we normally
see our patients every 4 to 8 weeks there may be a month where you do not visit
the office but the monthly fee is still due. To make this easier to track we
prefer post-dated cheques which can be processed monthly. Laraine,
our financial secretary, will be glad to discuss this with you at your first
visit and arrange a mutually acceptable fee schedule.
Many of our patients may not realize that their dental insurance has orthodontic
benefits or that they may have dual coverage if both parents have insurance
plans. We will be glad to fill out any necessary insurance forms for you to
make your claim process easier.
Unfortunately the insurance companies do not accept electronic claims like they
do for general dental treatment.
Payments may be made with cash, Visa, Mastercard, Interac or a personal cheque.
The following are guidelines from the Canadian Association
of Orthodontists to assist patients with their orthodontic insurance.
A dental insurance policy is a contract between
the insured and the insurance company. Our professional services are rendered
and charged directly to the patients account and the patient or person responsible
for the account is accountable for payment of all fees. Our office does not
accept pre-determination payments from the insurance company.
PLEASE BE SURE that you have submitted the ORTHODONTIC PRE-DETERMINATION FORM
which is the estimate outlining very important information that the insurance
company requires prior to submitting any monthly claims.
With each monthly remittance, please remember to:
COMPLETE one of your Standard Dental Claim forms in the usual manner. (Section
2 and 3 must be completed and signed by the insured)
ATTACH your paid receipt provided by
our office to your Standard Claim form with each monthly remittance.
It is very important that the paid receipt
is forwarded to the insurance company with each and every claim once treatment
has begun.
If you are unable to acquire insurance forms we will be pleased to provide
you with Standard Dental Claim forms as needed.
